This book examines the HIV/AIDS epidemic in the United States using the concept of syndemics to contextualize the risk of both well-known, and a few lesser-known, subpopulations that experience disproportionately high rates of HIV and/or AIDS within the United States. Since discovery, HIV/AIDS has exposed a number of social, psychological, and biological aspects of disease transmission. The concept of “syndemics,” or “synergistically interacting epidemics” has emerged as a powerful framework for understanding both the epidemiological patterns and the myriad of problems associated with HIV/AIDS around the world and within the United States. The book considers the disparities in HIV/AIDS in relation to social aspects, risk behavior and critical illness comorbidities. It updates and enhances our understanding of the HIV/AIDS epidemic in the United States and contributes to the expanding literature on the role of syndemics in shaping the public’s health.

Seeking international attention and development aid, its government faced a dilemma. Though devastated by conflict, Sierra Leone had a low prevalence of HIV. However, like most African countries, it stood to benefit from a large influx of foreign funds specifically targeted at HIV/AIDS prevention and care. What Adia Benton chronicles in this ethnographically rich and often moving book is how one war-ravaged nation reoriented itself as a country suffering from HIV at the expense of other, more pressing health concerns. During her fieldwork in the capital, Freetown, a city of one million people, at least thirty NGOs administered internationally funded programs that included HIV/AIDS prevention and care. Benton probes why HIV exceptionalism—the idea that HIV is an exceptional disease requiring an exceptional response—continues to guide approaches to the epidemic worldwide and especially in Africa, even in low-prevalence settings. In the fourth decade since the emergence of HIV/AIDS, many today are questioning whether the effort and money spent on this health crisis has in fact helped or exacerbated the problem. There is an inherently powerful and complex paradox underlying HIV/AIDS prevention—between the focus on collective advocacy mobilized to combat global HIV/AIDS and the staggeringly disproportionate rates of HIV/AIDS in many places. In Treating AIDS, Thurka Sangaramoorthy examines the everyday practices of HIV/AIDS prevention in the United States from the perspective of AIDS experts and Haitian immigrants in South Florida. Although there is worldwide emphasis on the universality of HIV/AIDS as a social, political, economic, and biomedical problem, developments in HIV/AIDS prevention are rooted in and focused exclusively on disparities in HIV/AIDS morbidity and mortality framed through the rubric of race, ethnicity, and nationality. Everyone is at equal risk for contracting HIV/AIDS, Sangaramoorthy notes, but the ways in which people experience and manage that risk—and the disease itself—is highly dependent on race, ethnic identity, sexuality, gender, immigration status, and other notions of “difference.” Sangaramoorthy documents in detail the work of AIDS prevention programs and their effect on the health and well-being of Haitians, a transnational community long plagued by the stigma of being stereotyped in public discourse as disease carriers. By tracing the ways in which public knowledge of AIDS prevention science circulates from sites of surveillance and regulation, to various clinics and hospitals, to the social worlds embraced by this immigrant community, she ultimately demonstrates the ways in which AIDS prevention programs help to reinforce categories of individual and collective difference, and how they continue to sustain the persistent and pernicious idea of race and ethnicity as risk factors for the disease.

Women experience considerable changes in their bodies, lives, and identity between the ages of twenty and seventy, including marriage, motherhood, the dissolution of relationships, and menopause, all of which often impact sexuality. In Deserving Desire, Beth Montemurro takes a wide-ranging look at the evolution of women’s sexuality over time, with a specific focus on the development of sexual subjectivity—that is sexual confidence, agency, and a sense of entitlement to sexual desire. Detailed stories of the ninety-five women in this study explore how they become more comfortable with their bodies, when most begin to enjoy sex, feel confident and positive about engaging in it, and how they become sexual subjects in control of their bodies. Deserving Desire explores the complex multi-stage process in which sexual subjectivity evolves over a woman’s lifetime. As girls, they learn about sex and how those around them—parents, peers, religion and media—regard sex. Physical and emotional transitions such as having a baby or ending a relationship further affect women’s sexual confidence and desire. Montemurro emphasizes that sexual subjectivity is about feeling in control of sexual decision making and acting purposefully and confidently. Though adolescent sexuality has been a major focus of sociological research, few studies have examined, as Montemurro does here, the development of sexuality through women’s lives and the events that change the way women feel about themselves, their bodies, and their relationships.
