Grâce aux progrès de la médecine et de la santé le nombre de personnes âgées est en nette augmentation. Le vieillissement, les troubles du comportement et/ou de la déglutition s'accompagnent d'évolutions physiques et physiologiques qui peuvent modifier l'observance médicamenteuse du patient et l'incidence des effets indésirables médicamenteux augmente avec la polymédication. Les objectifs de ce travail sont d'améliorer l'observance par une augmentation de la qualité de l'administration des médicaments et d'élever le niveau de connaissance des patients par un éducation thérapeutique adaptée permettant l'autonomie. Au cours de cette étude nous avons réalisé un audit clinique auprès des équipes saignantes sur l'administration des traitements dont l'aboutissement s'est orienté vers des outils d'amélioration pour aider les saignants dans leur pratique. Nous avons aussi mis en place des ateliers d'éducation du patient atteint de troubles bipolaires dans le but d'améliorer son autonomie et sa connaissance. Par la suite, l'audit clinique sera étendu à tout l'établissement de santé et les ateliers d'éducation thérapeutique seront continués pour permettre à d'autres patients d'être au centre de leur parcours de soins.
