Excerpt from A Treatise on Appendicitis In presenting a second edition of this book I have endeavored to bring the subject of the inflammatory lesions of the vermiform appendix up to the present state of knowledge. Professional interest in these lesions has not diminished, but rather increased, since the publication of the first edition. This circumstance, together with the fact that the latter has been so favorably received, decided me to comply with the request of the publishers to furnish the material for a second edition. It will be noted that much of the book has been rewritten, several chapters recast, and two new chapters added. In my experience with the operative treatment of these lesions, I have been struck with the difficulties attending the differential diagnosis even by physicians of acknowledged ability and repute. I have endeavored, in one of the new chapters, to so systematize available knowledge of lesions likely to be mistaken for appendicitis as to assist the practitioner at arriving at a conclusion in doubtful cases in as early a stage of the disease as possible, in order that prompt operative interference may be instituted at a time when the best results may be looked for. Realizing the fact that, in the very nature of things, some cases of appendicitis must of necessity receive non-operative treatment at some stage of the disease, and that occasionally cases must be treated during their entire course by so-called medical methods, a chapter has been devoted to this phase of the subject. The changes in the appearance of an organ brought about by disease are often manifested in such delicate deviations from the normal topog raphy that it requires the hand and eye of a genuine artist, and one who is also a well-informed anatomist and careful pathologist, to represent the morbid condition accurately. The sense of vision, when unaided, often fails to grasp the significance with completeness, and in such a case the true artist will palpate the fresh specimen and then represent the combined results of sight and touch. The superficial structure alone is frequently insufficient to demonstrate the character of the specimen, and the artist must find means of exhibiting the surface relations in association with the interior. This can be done: (1) by inserting lines, (2) by cutting away portions of the surface and thus displaying the depth, (3) by magnifying the organ and drawing it as though translucent, and (4) by explanatory diagrams, cross-sections.
